First, you need to get a regular deck of cards. Any deck will do, you don’t need to set them up a certain way or anything. Fan them out, and ask your target to pick any card. Make sure to tell them not to show it to anybody. Make sure you tell them to study it closely, so they can remember it. If there are lots of people, have them show the card to some other folks.
Here comes the sneaky part. While they are studying their card, take a quick peek at the card that you will place on top of it. Then when they put their card back into the deck (which you have conveniently open up for them) you’ll know which card is next to theirs. Then you can do whatever you want to “guess” their card.
What I usually do is sort through the deck, and alternate between looking at them, and looking at the cards, as if I’m trying to read their mind. Then when I get to their card, I quickly put it on top of the deck, with as little fan fare as possible. You can do this pretty easily with a little bit of practice.
Now you’ve got the deck with their card on top. You can do all sorts of fun stuff with. One way is to become distracted, and start talking about something else. Then divert their attention, and quickly take their card off the top and stick it under a napkin or something. Then later, say you forgot their card and ask them what it is. Look through the deck, and when you are unable to find it, ask them to look. While they are looking, you can “discover” the card under the napkin, or wherever you put it.
